Tape the printout to a piece of clear acetate / transparency film / thin clear plastic. (CD jewel-case lid works perfectly — clear, rigid, exact thickness.)

Score (lightly cut) the outline with a craft knife or scissors. Cut out all 4 trapezoids.

Tape the 4 trapezoids together along their 43.3mm slant edges. Form a 4-sided pyramid with 60mm × 60mm base, apex up. Use clear tape on the inside.

Place pyramid centered on iPhone screen, apex up, while running Holo. Lights off. Look at it from any side.

Pro tip — CD jewel case method: Take an empty CD case, snap off the lid (the clear plastic part). It's already the perfect rigidity, transparency, and thickness. Print this template, tape under the plastic, score with a knife, snap along the score. Done in ~3 minutes.
